Our goal is to reduce any barriers that someone living with HIV might experience to staying in medical care.

We know that getting and staying undetectable are the keys to staying healthy and reducing the risk of transmitting HIV to your partners. Undetectable = Untransmittable! It is our goal in Medical Case Management to support you and your doctor as you work together to reach that important “undetectable” status!

If you are HIV+ and meet the residency and income requirements, you can enroll in Case Management! Your case manager will be available to assist with all aspects of maintaining your health care – helping arrange transportation, appointments, medications, as well as accessible housing, substance abuse treatment, and mental health services, among many more services!
