A program designed to link patients to quality medical care
Healthy Connections is designed to link newly and previously diagnosed HIV positive patients to quality medical care and supportive services. Healthy Connections follows the ARTAS Strategy (Anti-Retroviral Treatment Access Services) and our ultimate goal is to assist patients in finding quality medical care that they can sustain for the long term – which helps both the individual and the community remain strong and healthy.
Who is Eligible?
Anyone who has recently tested positive for HIV or who tested positive previously and has not been in care for 90 days or more is eligible for Healthy Connections.
How does it work?
Dennis Escobar, our Linkage Coordinator, will meet with clients up to 5 times over the course of 90 days to develop and assist in implementing a plan to link them to care, ensuring that each client has transportation, understands the process of accessing care and what to expect at the provider visit, and accesses services that can be sustained long term no matter what the client’s income, insurance, employment or financial situation may be.
Client meetings will be held whenever and wherever the client is comfortable and safe. The utmost effort is made to maintain client confidentiality and privacy.
